When We Recommend a Tooth Extraction

Extraction is always a last resort. Dr. Englert and Dr. Sarkaria recommend it only after confirming the tooth can't be saved through a filling, crown, or root canal. Situations that may call for a tooth extraction include:

  • Unrestorable abscessed teeth where infection has compromised the root beyond repair
  • Advanced periodontal disease that has eroded the surrounding jawbone, leaving teeth loose
  • Deep fractures extending below the gumline into the root structure
  • Severe decay of both the exterior enamel and interior pulp that root canal therapy can't resolve
  • Extra or malformed teeth causing overcrowding, sometimes ahead of orthodontic treatment

For wisdom teeth, a CBCT scan can reveal their position, orientation, and proximity to nerve canals and sinuses well before symptoms appear. Identifying an impacted tooth early often means a simpler procedure and a faster recovery.

Simple & Surgical Extractions at Sycamore Dental

When a tooth is fully visible above the gumline and has a straightforward root structure, a simple extraction is typically all that's needed. We numb the area with local anesthesia, loosen the tooth from its socket, remove it, and pack the site with gauze.

Teeth that are broken at the gumline, impacted, or anchored by curved roots require a surgical approach. After administering local anesthesia, Dr. Englert or Dr. Sarkaria removes the tooth, debrides the socket, and places bone grafting material when indicated. The site is then sutured if needed.

Bone grafting at the time of extraction serves multiple purposes: it may help decrease the chance of dry socket, support new bone growth, and preserve the foundation for a future dental implant, bridge, or denture. This preparation happens during the same appointment rather than requiring a separate surgery later.

Every extraction begins with thorough local numbing so you typically feel pressure rather than pain. For patients with complex treatment plans or severe dental anxiety, our on-site board-certified anesthesiologists can administer general anesthesia, placing you in a fully unconscious state with a combined sedative and pain reliever. A consultation is required before general anesthesia can be scheduled so the anesthesiologist can review your medical history and plan the safest approach.

When a case calls for care beyond the scope of general dentistry, we refer to an oral and maxillofacial surgeon and send all imaging and records electronically so nothing has to be repeated on your end.

Many patients can resume normal routines within three to five days of a wisdom tooth extraction, though full recovery may take one to two weeks. Slight bleeding and swelling are expected in the first 24 hours and can usually be managed with gauze, ice, and over-the-counter pain relief. Our team provides detailed aftercare instructions before you leave.

Does a Tooth Extraction Hurt?
During the procedure, the extraction site is thoroughly numbed with local anesthetic, so you'll generally feel pressure rather than sharp pain. For patients who need it, general anesthesia is available on-site after a required pre-procedure consultation.

Will My Insurance Cover the Extraction?

What Is Dry Socket?
